As a Lifetime Member we will work with you to build a lifelong Circle of Support around your relative with a disability. We will do this by:
Completing a Focus Person Profile with you. This is a detailed account of the history of your relative with a disability; his or her gifts, goals, hopes and dreams for the future; your worries and fears about what could happen; family and friends who are actively involved; the people who play a supporting role in your loved ones life; hopes for future living; and financial arrangements and ways of securing plans for the future. This then gives the background information which may be needed to ensure the continuity and understanding of your relative's circle.
1. Completing an Equal Futures Expectations Form outlining your wishes for the future of your family member.
2. Recruiting and matching a Community Connector to your family. This will be someone who gets on well with your relative, has great interpersonal and group skills and is extremely community orientated.
3. Offering our workshops to you free of charge.
4. Support you and the Community Connector to form a lifelong network or Circle of Support around your relative.
The cost to become a Lifetime Member is £500 for year one. This pays directly for the hours worked by your facilitator and can be paid in one lump some or in installments over a 12 month period.
Costs for subsequent years are £60 membership plus the hours of facilitation that are are needed for the continuation of the circle (aproximately £150 in total).
All membership applications are subject to the approval of the Equal Futures Board of Directors , 60% of whom are Lifetime Members.
